皇甫姓源出有二：
1、出自西周。西周太师（高级武官）皇甫的后代以"皇甫"为姓，称皇甫氏。
2、出自子姓，是春秋时宋国公族的后代。西周后期宋戴公有个儿子叫公子充石，字皇父，宋武公时任司徒。当时有长狄鄋瞒部落进攻宋国，皇父领军反击，打退了长狄任，但皇父和两个儿子也不幸战死沙场。后来皇父的孙子南雍陲以祖父的字为姓氏，称为皇父氏。其六世孙皇父孟子，生子皇父遇。秦国灭宋时，皇父遇逃至鲁国。西汉中期，皇父遇嫡系子孙皇父鸾，自鲁迁居陕西茂盛陵，把姓氏中的"父"字改为"甫"字（古代二字同音通用），遂成皇甫氏。
